Dear Client,

Welcome to our clinic!We are glad you have chosen Let's Talk for your Speech/Language, Occupational therapy, tutoring and/or consultation needs.  We are sure that you will be satisfied with the exemplary services you will receive from us. 

All patients will need to download and complete the New Patient Forms.  Along with the New Patient Forms please download, complete and return the forms specific to the services you are seeking.  For example If you are a new patient for Speech therapy services, you will need to download the New Patient Forms as well as the Developmental History Form located under Speech Therapy Forms. 

In addition to the completed forms you will also need to bring the following items to your first appointment:

• Your current insurance and/or Medicaid card.
• Copy of your most recent Speech/Language, Occupational Therapy,Psychological or behavioral evaluations if you have them.
• Copy of current or most recent IFSP or IEP.
